This includes an improved relationship between the facial aesthetic subunits (specific regions of the face with defined boundaries), to diminish the signs of aging, or to improve your appearance after previous surgery.

Every person’s face is different, and so is the individual aging process. Therefore it only makes sense that each facelift is not performed the same way. A facelift result should look unique and natural to each patient. A facelift is the most encompassing surgical approach to correct sagging and facial wrinkles caused by aging. A facelift will remove loose excess skin, restore lost facial volume, and reposition underlying tissues and muscles of the face and neck. During a facelift mid face sagging, marionette lines, jowls, and a double chin are corrected.

NECK LIFT

The neck is commonly one of the first visible signs of aging. The lower jaw line, lower face, and the neck are important for a youthful appearance. A youthful appearance is perceived by a straight, defined jaw line, smooth taut skin and a well defined mento-cervical angle (angle between neck and chin on a lateral view). Slightly loose skin and lines of the anterior part of your neck, are early signs of neck aging. Most patients first notice these signs by the shadows on photographs below the chin and lower jaw line. Jowls, vertical banding of the platysma, horizontal lines, double chin or a fatty “turkey neck” below your chin add to an “aging” appearance. The neck, is the middle part of an aesthetic unit composed of the face, the neck and cleavage. If one of them does not match, the appearance will be perceived as disharmonious. It does not matter if this is the case before or after aesthetic surgery. Therefore it is important that all 3 of them are harmoniously balanced, and importance of the neck is not overlooked.

Many people have the impression that the fullness underneath their chin is only fat. Usually, it is a combination of loose skin, fat, and loose platysma muscles. Having a receding, narrow chin or mandible predisposes to the aging process by not providing bony support. Each patient should be examined and evaluated based on their individual anatomy and aging process. Fatty jowls, a “turkey neck,” vertical banding and horizontal creases all work together to make us look older than we often feel. A neck lift enhances the appearance of the neck by tightening the skin and underlying muscles and improving the contour of the jawline. This procedure is often performed in conjunction with liposuction, a facelift, or a cheek lift, but may also be done as a stand-alone procedure. When looking for facial rejuvenation, the correction of an aging neck with a surgical neck lift or other neck contouring procedures is pivotal.

CORRECTION OF THE DOUBLE CHIN

Many people are bothered by a double chin, and are looking for removal of the fatty fullness below their chin. Although a double chin is majorly composed of excess fatty tissue, a double chin is not always a sign of being overweight. A double chin can be just an inherited feature, running in your family, or it might be one of the first visible signs of aging. The correction of a double chin gives the patient a more youthful and vital appearance. A youthful appearance is perceived not only by a straight, defined jaw line, and a smooth taut skin, but also a well defined mento-cervical angle (angle between neck and chin on a lateral view). A double chin and loose skin efface a youthful appearing transition between your chin and neck. For some patients non-surgical procedures, for example, like Kybella® , Coolsculpting® or other non-invasive procedures might be a valuable treatment option. However, a non-surgical double chin correction, does not replace surgery, nor is it the best solution for every patient.

Eyelid Surgery

As gateways to the soul, the eyes are among the parts of the body that are quickest to show signs of aging. Small wrinkles, creases and fine lines form, while eye bags, discoloration and puffiness can appear in the lids. In some cases, the eyelids may droop as much as to obscure vision. Eyelid surgery can beautifully restore a balanced, pleasant-looking countenance while correcting any eyelid-related vision problems.

Eyes are a very important facial feature. They reflect our mood, they express our feelings. As we age though, quite often their look doesn't correlate with what we actually feel. Patients that come in office seeking for improvement on how their eyes look often describe themselves as looking tired even when they are not. A lot of them also feel the upper eyelids heavy, as if they negative effect their vision. Eyelid bags on the other hand are common even at a younger age.

Patients can not be fitted in just one category and individual characteristics play an important role on choosing what to do, how to make things better. That's why pictures and discussion are crucial.

Transconjuctival Approach                Classic Approach

suitable for younger patients             treats skin laxity

no external incision                           hidden skin incisions

shorter recovery                               needs 7-10 days

On younger patients the main problem is the lower eyelid bags. Patients usually mention that someone relative also has the problem too. The operation of choice is trans conjunctival approach. The approach for these eyelid bags is an incision through the conjunctiva without any external visible mark. The recovery is quick, it usually followed by some bruising. The skin on these patients is of good quality so in the next few weeks it shrinks and there is no need for further intervention. Occasionally a minor laser resurfacing may be necessary a couple of months later. The anesthesia of choice is general and the procedure lasts about 30 min.

On older patients we need to make an external incision that will remove the extra skin. A manipulation of the lateral canthus is also a part of the surgery to make the eyelid tighter. The recovery is a bit more prolonged as bruising and swelling are more of an issue.

As far as the upper eyelid is concerned we focus on excising a minimal amount of skin and muscle. This along with the removal of the fat located close to the nose are enough to reshape the eyelid. Patients sometimes ask for an upper eyelid vlepharoplasty in order to fix the lowering of their brow as they age. Unfortunately vlepharoplasty will only modestly improve this problem. Use of botulinum toxin or proper brow surgery can give a better result on these patients.

Although the worse complication of vlepharoplasty is considered to be blindness this is sporadically reported. When patients are well informed and have good communication with their surgeon this will never happen. Excessive swelling combined with pain in the first 6 hours post operative is a cause of alert. Minor ectropion or chymosis can present after the operation but rarely they need anything else than medication and some physiotherapy.

As in every operation realistic expectations and a good surgical plan are necessary in order to get the best result. Careful planning for adequate recovery is fundamental.

CHIN SURGERY

The term chin surgery, mentoplasty, or genioplasty are often used simultaneously for changing the shape of your chin. This means chin advancement, chin augmentation, or chin reduction in a vertical or horizontal plane. Chin surgery is often performed to improve the proportions of the face. The chin plays an integral role during the consultation of rhinoplasty patients, patients seeking a face lift / necklift, or those seeking a correction of their double chin. Our Patients seeking chin surgery or a profile-plasty are always assessed for their occlusion. Some patients who are seeking chin surgery will require orthognathic (corrective jaw) surgery for functional and medical reasons. Patients seeking less lengthy and invasive surgical methods to change the shape and size of their chin often commit to chin surgery only. The goal of chin surgery is to improve the facial proportions mainly from an aesthetic point of view. Chin surgery is an integral part of a profile-plasty. The surgical outcome of patients seeking a facelift, neck lift or a double chin correction can be taken to the next level, in some patients, by performing an additional chin surgery.

Brow Lift

A low-hanging, wrinkled brow can cause others to perceive you as tried, disgruntled or even angry, even when you’re in the best of spirits. It may also obscure vision in some cases. The brow lift procedure can help smooth out the forehead and elevate the eyebrows, creating a more well-rested and pleasant countenance
